Servo Fuel Injector

Recently my engine has been running real rich, requiring leaning the mixture to get it to smooth out. The engine also will not shut off when the mixture is pulled full lean. I have to use the key. The fuel pressure is right where it should be. The fuel line AD was just done in December, so I'm wondering if it could be anything related to that. My servo was pulled this weekend so it can be sent in to be checked, and rebuilt if needed. It's sounding like a bad o-ring or diaphram in the servo, letting too much fuel through.
Have any of you had your servo go bad? Any other thoughts of what could cause this?
